On July 8, 2005 the FDA ordered warnings be put on the package insert labels of Viagra, Cialis, and Levitra to the effect that some users of these impotence drugs -- or erectile dysfunction (ED) drugs -- have developed NAION, a form of blindness. At the same time, however, the FDA stated that it is impossible at present to know if NAION is a side effect of these ED drugs, or an unrelated medical condition.
NAION is considered one of the most common causes of sudden vision loss in older people, with anywhere from 1,000 to 6,000 cases a year, also NAION is one of the most common causes of unexpected or sudden vision loss in older Americans. A new study from the Brady Urological Institute at Johns Hopkins suggests an over-supply of a simple blood sugar could be a major cause of erectile dysfunction in diabetic men.
Researchers have found that one particular simple sugar, present in increased levels in diabetics, interferes with the chain of events needed to achieve and maintain erection and can lead to permanent penile impairment over time. The results, which have implications for new types of erectile dysfunction treatments targeting this mechanism of erection, are described in the August 16 issue of the Proceedings of the National Academy of Sciences.
Previous research had shown that diabetic erectile dysfunction was partially due to an interruption in an enzyme that starts the chain of vascular events leading to an erection. The Hopkins team suspected O-GlcNAc, a blood sugar present in hyperglycemic (high blood sugar) circumstances, to be that interrupting factor.
"We were interested to determine whether high glucose in mellitus modifies the endothelial nitric oxide synthase (eNOS) enzyme, which is responsible for the achievement and maintenance of erection," says Biljana Musicki, Ph.D., lead investigator of the study and a research associate in the Brady Urological Institute.
Erectile dysfunction is a common problem for more than half of men with diabetes. Musicki says that an estimated "50 percent to 75 percent of diabetic men have erectile dysfunction to some degree, [a rate] about threefold higher than in non-diabetic men." This is not the same type of erectile dysfunction seen in non-diabetics, and it is less effectively treated with conventional drugs like Viagra.
The study examined rats with type 1 mellitus as well as the overall mechanism of erection. "Erection begins when a sexual stimulus activates the enzyme neuronal nitric oxide synthase (nNOS) that causes short-term release of nitric oxide (NO) at the nerve endings in the penis," Musicki explains.Read More>>

UPDATE 1-Icos quarterly loss narrows on Cialis sales
NEW YORK, Aug 4 (Reuters) - Icos Corp. (ICOS.O: Quote, Profile, Research) on Thursday reported a smaller quarterly loss as growing sales of Cialis, the erectile dysfunction drug it shares with Eli Lilly and Co. (LLY.N: Quote, Profile, Research), helped offset marketing costs.
The biotechnology company posted a net loss of $22.6 million, or 35 cents per share, compared with a loss of $51.9 million, or 82 cents a share, a year ago.
Analysts on average expected a loss of 38 cents per share, according to Reuters Estimates.
Bothell, Washington-based Icos said it expects its full-year net loss to be $1.20 to $1.28 per share.
The company forecast 2005 worldwide Cialis sales of $775 million and said it expects selling, general and administrative costs to decline in the second half of the year.
Sales of Cialis, the Viagra rival that boasts effectiveness for up to 36 hours, rose 39 percent to $190.9 million for the quarter.
Icos revenue for the second quarter was $18.1 million, up from $17.9 million a year ago.
Marketing costs rose to $10.5 million, from $10.1 million a year ago.

FDA Scientist Warned of Viagra-Blindness Link: Report
More than a year before news broke that the erectile dysfunction drug Viagra was possibly linked to a rare form of blindness, a U.S. Food and Drug Administration safety officer alerted superiors to the possibility, the Washington Post reported Friday.
The unidentified safety officer had been monitoring adverse event reports and told supervisors in April 2004 that doctors and users should be warned of the findings, she reportedly told congressional investigators. But the FDA issued no public warnings or changes to the drug's label until a scientific journal reported last month of the possible link, the newspaper said.
On Monday, Pfizer issued a statement saying it had no evidence in trials involving 13,000 people of a link between Viagra and a form of blindness called non-arteritic anterior ischemic optic neuropathy (NAION). That followed a report in last month's Journal of Neuro-Ophthalmology, which examined seven cases where men suffered NAION within 36 hours of taking Viagra, the Post said.
The FDA has said it has 38 reports of NAION among Viagra users and five additional reports among users of other impotence drugs, the newspaper said.
In response to the latest report, Consumer's Union, publisher of Consumer Reports, renewed a call for congressional reform of the FDA's drug monitoring system. "The fact the agency responsible for drug safety knew for a year that Viagra was linked to blindness, but did nothing with that information, is a clear indication that the system is broken," a spokesman said in a prepared statement.

Pfizer said it is the company's understanding that the FDA has asked that similar information be included in the labels of all oral erectile dysfunction medicines. The FDA reported there were instances of NAION among those taking Cialis as well as Levitra.
Pfizer said that a review concluded that Viagra doesn't increase patients' risk of blindness but that it is still working with federal regulators to update the drug's label to reflect rare reports of vision loss.
Last month, the U.S. Food and Drug Administration said it had 38 reports of blindness caused by a a condition called NAION or non-arteritic anterior ischemic optic neuropathy among users of Viagra.

What is Silagra? Silagra is the brand name for Sildenafil Citrate. It’s a medicine that helps men with erectile dysfunction (ED) have sexual intercourse again.
